ABOUT THE PROJECT

The transitional care project is devoted to enhancing and supporting current UCLA efforts from pediatric attendings and residents to solidify a transition system for adolescents transferring to adult care. This involves educating adolescents on health education while also empowering them to take steps towards independence and health autonomy.

We are also committed to providing resources to assist teens with disabilities, as well as fostering discussion in the community, educating parents on how to facilitate this transfer of care, and working with pediatric healthcare providers to utilize resources to promote transition in their patients.​ Click below to explore our resources and learn more!

RESOURCES

We have resources for both adolescents and adults. Our resources for adolescents are focused on being interactive and engaging so that adolescents may explore transitional care for themselves. Our resources for adults are primarily for parents and guardians who wish to learn more about the process of health transition and the many ways in which they can support their transitioning child.​
