The Health Passport provides adolescents with a tool to keep track of their health information as they age and transition into adult care. As adolescents grow up and become more independent, it is important that they become familiar with their specific health information, which includes insurance coverage, prescriptions, allergies, past medical history, and more. This is especially important as California privacy laws gradually restrict parents’ access to their children’s health information before they turn 18 years of age.

The Health Passport helps to empower adolescents in taking charge of their health earlier on in their lives so that they can continue to seek healthcare services as independent young adults.
