Age 12-13

Encourage your teen to learn about their own health and ask their doctor their own questions.

18-21

Your child is a legal adult and legally responsible for their own care. Make sure your young adult keeps a copy of their medical summary and always carries their health insurance information with them.

14-15

Help your teen become more independent: support them to carry their own health insurance card, make appointments, and speak to their doctor alone.

22-25

Your young adult should get care from their adult doctor, learn to manage their own health and health care, and update their medical summary.

16-17

Encourage your teen to take ownership over their healthcare, and start preparing for the transition: talk about privacy rights, determine if they need new doctors, and figure out the best way to support them.
