Studies have shown that late adolescents and early adults have poorer health outcomes than adults in their mid-twenties. This is partly because of a lack of adequate resources to help healthcare-transitioning youth in taking ownership of their health.
The process of transitioning requires taking on new responsibilities and can seem overwhelming.
